JOB description

Care Coordinator, West Virginia Army National Guard (WVANG)

Location: South Charleston, WV

Full-Time Position, Days, Monday - Friday, occasional weekends for special events


NOTE: This Position is contingent upon contract award


Purpose: To provide Medical  Non-clinical Medical Case Management in support of Individual Medical Readiness (IMR) to the WVARNG


Duties:

  • Obtain and maintain access to and utilize the following military medical systems: AHLTA; MODS: eProfile, MEDPROS; and MEDCHART: eCase, eMMPS, HRR. 
  • Input medical charting and maintain referrals indicated in MEDCHART. 
  • Provide Care Coordination for all assigned Soldiers of the WVARNG. 
  • Manage Soldiers’ medical documentation throughout the continuum of care from the identification of a condition to when the Soldier is either considered medically ready to deploy or is administratively separated from the military. 
  • Obtain completed release of medical information forms from Soldiers as required and submit these forms to the Soldiers’ medical provider in whatever manner they require (fax or hardcopy) to obtain all pertinent documentation. 
  • Maintain awareness of, and work with individual Service Members (SM), their unit command teams, and the G1 Human Resources department to track the status and documentation of any and all referrals for medical care, to include: Line Of Duty Care referrals from MEDCHART: eMMPS; MODS: Post-Deployment Health Assessment and Health Re-Assessment; and DOEHRS Hearing Conservation program. 
  • Track medical appointments by communication with the Soldier and/or his/her unit to ensure Soldier attendance and submission of follow-up medical documentation. Contractor shall obtain medical documentation if not provided by the SM. 
  • Log in eCase any contact with case managed SMs and/or their unit, to include: email, voice call, personal interview, etc. 
  • Submit case processing, medical documentation, and profile concerns to the State Surgeon or his designee as soon as identified, either in person or by email. 
  • Ensure all medical documentation received from SMs and/or their medical provider(s) is received, scanned, and uploaded into MEDCHART: Health Readiness Record (HRR), and is properly indexed once uploaded. All documentation will also be retained in hardcopy in the Soldier’s Service Treatment Record (STR). 
  • Maintain Soldier medical records and Service Treatment Records (STR) in accordance with Army Regulation (AR) 40-66. 
  • Maintain awareness and working knowledge of all applicable Army Medical Department regulations, policies and publications applicable to Individual Medical Readiness (IMR), as well as applicable Combatant Command Medical Deployability guidance. 
  • Attend and assist with PHA and SRP events as required. 
  • Run MEDPROS reports before and/or after PHA/SRP events as required. Ensure all scheduled services are completed/updated and current medical status is reflected.
  • Review and update health assessment (PHA, SRP) lab results in electronic and hardcopy medical records. 
  • Perform MODS MWDE data entry of any and all required medical information to include: immunizations, designation of pregnancy, vision readiness classification, medical warning tags, HIV blood draw date, lab results, etc. 
  • Educate Soldiers on resources available to improve medical and dental readiness. 
  • Abide by Privacy Act, HIPAA regulations, and guidelines. 


Qualifications


High School Diploma

Prefer one or more years’ experience as a Care Coordinator or in a similar role

Knowledge of Microsoft Office suite

Strong organization, verbal and written communication skills


Must be a U.S. citizen and have the ability to pass a background check/security clearance.


Staffing and Scheduling:  

* Unless otherwise specified, the Care Coordinator shall perform the activities and services required during normal working hours established by the requiring organization. Normal working hours is defined as eighty (80) hours, every two weeks.
